Injured and Ill Employees

Short-term disability (STD) is an employee benefit that replaces some of your income if a personal injury or illness prevents you from working. While you are away from work, it pays you a certain percentage of your income for a set period of time. Your human resources department is usually the best source of information about specific plans. To obtain short-term disability benefits, call (800) 354-6323; fax your name, address, and telephone number to (517) 327-2542; or e-mail your name and address to idm@fincorsolutions.com. You can also apply for benefits online.

How Soon Will I be Able to Obtain Short-term Disability Benefits?

Generally, you will be able to receive benefits within 14 days of receiving the completed STD claim form. Benefits are paid bi-weekly.

When You Are Injured at Work

If you feel you have injured yourself at work, you must contact your human resource’s department. You will be asked to complete an incident report, and FinCor Solutions will be notified that you have applied for workers’ compensation benefits. If these benefits are denied, we will request that you complete a reimbursement agreement as well as your STD claim form. Once we receive the reimbursement agreement and STD claim form, we will issue payment.

What Else Should I Know?

  • Once you have received your claim form, you will need to complete the top portion of the form and sign it. Please have your physician complete the bottom portion. Once completed, return the claim form in the envelope provided or fax to (517) 327-2542.
  • Upon receipt of the completed claim form and the approval of the carrier, your claim will be processed in time for the next regular payroll run.
  • Please be sure to update your FinCor Solutions’ representative with any changes or details of your disability leave to avoid a lapse in payment or possible reimbursements.
  • Short-term disability benefits are taxable. Your year-end W2 will come from your employer.
